IU Human Research Protection Program - Affiliate Account Request Form

This form is intended for Indiana University (IU) affiliates who require access to the IU Institutional Review Board (IRB) online protocol submission system, Kuali Protocols. If your access needs pertain to other IU systems or resources, please coordinate with your IU collaborator to obtain the appropriate access.

To access Kuali Protocols, an active Indiana University (IU) computing account (username and password) is required. This account is necessary if you are involved in creating, submitting, or managing research projects within the Kuali Protocols system-either independently or on behalf of others. It is also required for individuals serving as Principal Investigators on studies reviewed by the IU IRB.

To request an account, please complete the information provided below. Upon receipt of your request, the Human Subjects Office will evaluate your need for access and, if appropriate, submit a formal account request on your behalf. Further instructions will be shared once the account has been established.
