Thank you for your interest in the PEARL Study at Purdue. To determine if you are eligible to participate, we ask that you complete the few questions below.
Please note: This study requires there be an ambassador (parent, guardian, significant other) who participates along with the person with autism. You may fill out this questionnaire as the potential participant (person with autism) or their ambassador. We will refer to the potential participant as the "participant" throughout the questionnaire.
